Restoring changes all the changes in MOODLE_16_STABLE since MOODLE_16_BETA.

This had to be done because the MOODLE_16_STABLE branch tags were moved.

ROLES AND PERMISSIONS  - FIRST CHECK-IN

WARNING:  DEV IS CURRENTLY VERY UNSTABLE.

This is a mega-checkin of the new Roles system.   A lot of changes have
been made in core and modules.

Currently there are a lot of rough edges and known problems.  We are
working hard on these .. .the reason for getting this into HEAD at this
stage is enable us to move faster (our branch was diverging from HEAD
too much).

Please keep an eye on http://docs.moodle.org/en/Roles for current status
and information for developers on how to use the new Roles system.

Added capabilities_cleanup() upon block deletion.

Added hooks so that blocks can create functions that are run when they are installed and uninstalled. The base implementation does nothing.

Revisited blocks administration page!

   * Incompatible blocks which have been disabled during the upgrade
     from Moodle 1.4 can now be deleted.

   * Incompatible blocks (if any) are presented in a separate table
     and are clearly marked as such.

   * Using the new table class.

   * Improved visuals.

   * Fixed HTML and updated obsolete coding conventions all around.

The admin blocks page now doesn't crash when encountering an incompatible block;
instead, it shows a notice.

Improved error handling a bit.

Removed the "hardcoded" method to find the block name, the correct is to ask
the block itself. It was causing errors for blocks with no lang file. Also,
removed some useless commented code.

Added two strings about deleting blocks from admin interface...

Changed the way the fix for bug 1255 works, while making sure it still works :)
It just didn't feel right to intrusively set object properties from "outside",
and since it's now one variable assignment less, I thought why not... :)

Solved one issue that caused the show/hide option to fail.

First Official realease of the "Blocks System".
Integrated with Moodle:
    - Install automatically.
    - Update course->blockinfo for each course at installation time.
    - Modified course creation to insert into blockinfo field.
    - Modified restore to insert into blockinfo field.
    - Admin options (hide, show, delete, config) from admin page.
    - Lang strings inserted (en only).
    - Database support to mysql and postgresql (not tested!!).
Moodle, backup and block versions updated to 2004041800.
Tested with mysql: Install fresh and upgrade from previous.
section_activities block renamed to social_activities and created
its own lang file to support name "Social Activities". This can
be changed at any time.
